COOPERATIVES REGULATIONS 2003 (NO 22 OF 2003) - REG 20

The following particulars are prescribed:

        (a)     the name of the cooperative;

        (b)     the address of—

              (i)     the cooperative's registered office; and

              (ii)     the cooperative's principal place of business;

        (c)     the name, address and position of the person sending the annual report to the registrar;

        (d)     the date of the report;

        (e)     the number of persons employed full-time and part-time by the cooperative at the end of the relevant financial year;

        (f)     the number of persons who performed voluntary services for the cooperative in the relevant financial year;

        (g)     the number of members in the cooperative at the end of the relevant financial year;

        (h)     the number of shares forfeited under the Act, part 6 in the relevant financial year;

              (i)     the number of memberships cancelled under the Act, part 6 in the relevant financial year;

        (j)     the number and class of shares issued to members during the relevant financial year;

        (k)     the date of the annual general meeting.
