        (c)     the tag includes 1 or more of the following:

              (i)     the name and address of the cat's keeper;

              (ii)     the name and address of the cat's carer;

              (iii)     a contact telephone number for the cat's keeper;

              (iv)     a contact telephone number for the cat's carer; and
