WORK SAFETY REGULATION 2009 (NO 45 OF 2009) - REG 178

A construction induction training card must contain the following details:

        (a)     the applicant's full name and date of birth;

        (b)     the applicant's signature;

        (c)     the date of issue of the card;

        (d)     a unique identifying number for the card.
