(1) A compensation
application must be —
(a) made
in writing on a form approved by the Chief Assessor; and
(b)
given to the Chief Assessor.
(2) If a person
entitled to make a compensation application is —
(a)
under 18 years of age — the application may be made on his or her behalf
by a parent, or a person acting in place of a parent, of the person;
(b) a
represented person within the meaning of the Guardianship and Administration
Act 1990 — the application may be made on his or her behalf by the
person’s guardian or administrator appointed under that Act.
