A written prescription for a medicine must—
(a) be signed by the prescriber; and
Note The prescription must be signed with the prescriber's usual signature (see Act, dict, def signs ).
(b) if the prescriber amends the prescription—be initialled and dated beside the amendment by the prescriber; and
(c) be written in terms and symbols used in ordinary professional practice; and
(d) if the prescription is for an unusual or dangerous dose—include the prescriber's initials beside an underlined reference to the dose.
