PUBLIC HEALTH (INFECTIOUS AND NOTIFIABLE DISEASES) REGULATIONS(AMENDMENT) (NO. 27 OF 1993) - REG 9

    (a)     by omitting Form 1 and substituting Form 1 set out in the Schedule to these regulations;

    (b)     by omitting Form 2 and substituting Form 2 set out in the Schedule to these regulations; and

    (c)     by adding at the end Form 4 set out in the Schedule to these regulations.

Schedule 2
