MEDICINES, POISONS AND THERAPEUTIC GOODS REGULATION 2008 (NO 42 OF 2008) - REG 55

A written requisition for a medicine must be—

        (a)     signed by the person (the issuer ) issuing the requisition; and

Note     The requisition must be signed with the issuer's usual signature (see Act, dict, def signs ).

        (b)     if the issuer amends the requisition—initialled and dated by the issuer beside the amendment.
