MEDICINES, POISONS AND THERAPEUTIC GOODS REGULATION 2008 (NO 42 OF 2008) - REG 56

A requisition must include the following particulars:

        (a)     the name of the person issuing the requisition;

        (b)     the capacity in which the person is issuing the requisition;

        (c)     the date the requisition is issued;

        (d)     the medicine, and the form, strength and quantity of the medicine, to be supplied on the requisition;

        (e)     the pharmacy or ward to which the medicine is to be supplied.
